Never before you've seen your site. I build the demo first, for free. A deposit is only taken once you've approved the design, usually around day four, and the balance is due when you go live.
Yes, all of it. The website, the code, the domain and the content are yours to keep. You are not renting a template and you are not locked into anything. If you ever want to move it or hand it to another developer, you can.
Not at all. It's there if you want a set-and-forget arrangement, but plenty of people take their site and run it themselves. Either way works, and you can start, stop or skip it with 30 days' notice.
